Remove obsolete changelog instruction from template

The template's opening comment (lines 4–5) still instructs contributors to add a bullet under CHANGELOG.md's Unreleased heading, but this PR removes that heading and retires the separate changelog. Remove that instruction too so contributors are not directed to recreate the changelog.
